ORDERING INSTRUCTIONS

1. Placing your Order

   Follow link to the order form and print the form. You can order by calling the  number above (Monday through Friday, 9:00 AM to 5:00 PM EST), by Faxing the printed form to our Fax number, or by mailing the printed form and payment to our address above.



2. Sold To/Ship To

   Complete "Sold To" and "Ship To" addresses.  A street address is required for the "Ship To" address; UPS will not ship to Post Office Box numbers.



3. Items

   Fill out all applicable order blanks completely and mail in an addressed envelope to the Impressive Imprints address above. Lack of information may delay processing your order. When ordering, make sure to include the style number, color, description, size, and exact quantity of each item.
   
   

4. Shipping Cost

   All orders are shipped UPS Ground or U.S. Postal Service. 2nd Day Air and 3-Day Select shipping is also available.
     *Please call for current shipping rates.
   **Equipment is usually available Game Days at the Adirondack Broomball Rink.
   

5. Payments

   Each order must have one of the following payments:
   
   1. Full Payment With Order: Enclose a Check or Money Order.
   2. Payment By Credit Card: We accept MasterCard, VISA and American Express.



6. Delivery Information

   Complete order information and payment is essential to begin processing your order. Impressive Imprints will attempt to meet all stated delivery schedules, but assumes no liability for delays due to our inability to obtain raw materials, supplies, or labor; or due to conditions over which we have no control, including, but not limited to, delays of suppliers or carriers, acts of war, nature, or any other causes.
